In 2009 one of Toie 6000 was involved in an accident at Kebon Pedes with BN-Holec resulting in the cab needing to be rebuilt. The shortage of original spare parts, made Balai Yasa Manggarai rebuild it in a creative way. Originally, the cab design was based on bullet-like.
